Chronic Care Management

Our CCM program is particularly useful for managing two or more chronic conditions that are expected to last for a year or more, and includes services such as a dedicated Care Manager, care plan development and treatment goals, biometric remote monitoring, on-demand physician visits, and 24/7 access to care and health information.
Telehealth
Our telehealth services enable us to provide convenient and secure virtual consultations, remote patient monitoring, online appointment scheduling, and quality primary care services, allowing patients to receive personalized healthcare from anywhere with an internet connection.
Vis-À-Vis telehealth services are already available 24/7 across New York, New Jersey, Pennsylvania, and Florida, and are expanding into North Carolina, South Carolina, Connecticut, Massachusetts, Georgia, and Kentucky soon.
Care Coordination

We help to coordinate orders for in-home various ancillary service providers with referrals to ambulatory surgery centers, ambulatory PACUs, infusion centers, imaging centers, outpatient endoscopy services, cardiology offices, orthopedic offices, and specialists.
Behavioral Health Management
Our Behavioral Health Management services are designed to help individuals effectively manage mental health conditions.
